Introductory Psychological Exam

1. Do you have any physical problems that you are concerned about, seeing a doctor about, or taking medications for?
*If yes, what are you concerned about?
2. Do you feel depressed, sad, or burned out?
*If yes, have you ever had any feelings of wanting to harm yourself?
*Have you ever tried to harm yourself? If yes, when and how?
3. Do you have a problem with anger, such as having a temper?
4. Do you resent being given advice?
5. What do you like to do for leisure or free time?
6. Do you think that people are basically not very trustworthy and would take advantage of you if they could?
7. Are you sensitive to what others think of you?
8. Are you anxious or nervous?
9. Do you feel separated from other people like it is you against the world?
10. Do you like to stay active and get bored easily?
11. Are you uncomfortable in social situations, like being in groups or meeting people for the first time?
12. Are you aware of being afraid or fearful of anything specific?
13. How does the future look to you at this time?
